The Opportunity

New Horizons is seeking passionate individuals that are keen to apply their enthusiasm, energy and outgoing personality in a leading Lifestyle Planner role. As a member of the Customer Engagement team, you will report to the Customer Engagement Manager and be instrumental in increasing our customers' engagement in the planning and delivery of their supports, whilst promote choice and providing exceptional customer service.

Your working days and hours are Monday to Friday from 8:00 AM to 4:30 PM.

Your responsibilities will include (but not limited to):

  • Supporting team members and stakeholders by sharing knowledge, information and expertise
  • Capturing and documenting customer information and ensure documentation  is maintained
  • Working alongside customers, their families and decision makers to increase participation and inclusion based on the customers goals and choices
  • Developing new business opportunities to achieve customer acquisition targets
  • Documenting clear and concise records, and submitting monthly reports

Skill and Experience:

  • Experience in developing complex support plans
  • Experience in Disability and Mental Health is considered as an advantage
  • Experience in support facilitation/coordination
  • Demonstrated planning, time management, organisational and problem solving skills
  • A current NSW Driver Licence and own vehicle
  • Willingness to travel between site locations and Regions as required
  • Willingness to obtain an Australian Federal Police Check, Working with Children Check (WWCC)  that meet New Horizons employment criteria prior to commencing with us
