The Opportunity

We're looking for a person who is passionate about supporting our customers to achieve their goals and increase their independence. The role involves supporting customers to engage with the community as well as improving practical living skills.

The position is 24 hours per week and based in North Ryde. The role reports to the team leader within our Supportive and Caring team.

Key responsibilities include:

  • Develop, implement and review individualised support plans
  • Develop strong relationships with key stakeholders
  • Document clear and concise records
  • Utilise customer funds that help them achieve their goals
  • Working in close collaboration with the team leader and support staff to ensure positive customer outcomes

 

Skill and Experience:

  • Demonstrated experience in working with people with a disability/ relevant human services industry
  • Sound knowledge of and experience in coordinating with local services
  • Capacity to relate to people from differing cultural backgrounds
  • Well-developed understanding of work health and safety practices
  • Certificate IV in Mental Health/Disability Services
  • Australian Federal Police Check, WWCC, driver's licence and current first aid certificate
