The Opportunity

We're looking for an experienced Coordinator to provide specialist customer support on a full time, fixed term basis for 8 months. This innovative role helps breakdown the barriers and simplifies support coordination for our customers.

You will assist our high needs customers to connect and access supports that help them on their recovery journey.  

Working in the Specialist and Advisory team, you will report to the Clinical Manager for the Greater Sydney region.

Your working days and hours will be Monday to Friday, 8am to 5pm.

Key responsibilities include:

  • Formulating and implementing customer plans and adapting to their changing needs
  • Creating a collaborative team environment that aligns to New Horizons' values
  • Providing innovative solutions to enhance customer experience
  • Developing strong relationships with key external and internal stakeholders
  • Maintaining accurate and up to date records and provide timely reports to relevant stakeholders

 

Skill and Experience:

  • Working knowledge of Human Rights and Person Centered Practices under a recovery focused approach
  • Understanding of the NDIS operating model
  • Well developed knowledge of and experience in coordinating with local services
  • Relevant tertiary qualifications in  either Mental Health or Disability Services
  • AHPRA registration to relevant professional body
  • Hold an Australian Federal Police Check (from the last 6 months), Working with Children Check for Employment and a Driver Licence
